INITIAL VISIT
Your initial visit at Pastagia Periodontics is our time to get to know each other and see how we can best help you achieve your oral health goals. The more information we can gather ahead of time, the more streamlined your first appointment will be. We ask that you have any previous x-rays emailed to our office at info@pastagiaperio.com
 
During your initial consultation, we will take any additional necessary dental x-rays and do a complete periodontal examination to assess the health of your gums and underlying bone. We will also consult with your general dentist in order to formulate a customized treatment plan to address your specific needs and concerns.

INSURANCE
If you would like us to help you maximize your dental benefits, please make sure to fill out all of the information in the insurance section on our Patient Registration Form, including the group number and ID number found on your insurance card.
It would also help us a great deal for you to e-mail us a photo of the front and back of your insurance card, and include the primary subscriber’s full name and birth date.
 
Payment in full is expected at the time services are rendered. Payment can also be made online. As a courtesy, we will file claims to your dental benefits carrier on your behalf to help you get reimbursed by your dental benefits carrier quickly and easily.

NO INSURANCE? NO PROBLEM!
We accept all major credit cards, cash, and check. We also offer no-interest payment plans.
